If a sale is cancelled after the title paperwork has already been sent to the DMV, the following paperwork is needed for submission to the DMV for cancellation.
- Title (if the title is still under review by the State, this can be skipped)
- Signed Title application
- PS2025B - Affidavit to Correct Ownership of a Record
- Lien release (if applicable)
- Refund request form for sales tax - Sales tax is refundable up to 90 days from the purchase date
